Demysifying AHPRA: Which Registration Pathway is Yours

Australia isn’t just about picking a nice city.

AMC, ECFMG, EPIC, PESCI, the acronyms alone are enough to cause a headache. The most common mistake IMGs make is treating “registration” as one single step rather than a staged sequence. If you dive into paperwork without identifying your correct pathway, you risk hitting a wall.

For example, some pathways require a firm job offer before you can even sit for your Pre-Employment Structured Clinical Interview (PESCI). Getting this sequence wrong means your application gets kicked back, delaying your start date and your income.

The Four Core Pathways

The secret to a fast relocation is choosing the right pathway and executing the steps in the correct order. Here is how the Medical Board of Australia structures your entry:

1. The Competent Authority Pathway

For IMGs who have trained or practised in recognized systems (like the UK, Canada, or New Zealand). This is generally the most streamlined route to general registration.

2. The Standard Pathway

For doctors whose qualifications are not eligible for the Competent Authority route. This requires passing the AMC exams (MCQ and Clinical).

3. The Specialist Pathway

for overseas trained specialists with recognised transferable equivalent skillsets, applying to the relevant college for recognition in Australia and then 12 months minimum of supervised practice.

4. Expediated pathway

This new pathway as of 2025 is offered to NZ, UK, and Irish Doctors who have a specialist fellowship in General Practice, Psychaitry, Dermatology, or Obstetrics and Gynaecology. This requires 6 months mentorship and application is via AHPRA directly

  • AMC Portfolio: Establish portfolio on AMC website and link to EPIC.

2. Assessments

  • English Language: Pass IELTS, OET, or PTE (must be current).
  • AMC MCQ: Study for and pass the Computer Adaptive Test.

3. Employment & Interview

  • AHPRA Application: Submit application + Supervised Practice Plan (SPPA-30).
  • Visa Application: Apply (usually 482 visa) once registration is approved in principle.

4. Registration & Visa

  • AHPRA Application: Submit application + Supervised Practice Plan (SPPA-30).

5. Launch

  • ID Check: Present in person at AHPRA office upon arrival. (Can be done remotely)
  • Application for provider number: Apply for Medicare Provider Number (PRODA account required).

Securing the Right Job and Mastering Australian Billing

Australia isn’t just about picking a nice city.

You’ve navigated AHPRA and you’re ready to fly. But finding a job in As an IMG, you are immediately faced with Section 19AB of the Health Insurance Act, which dictates where you can work to access Medicare rebates.

If you don’t understand the Distribution Priority Area (DPA) classifications or the 10-year moratorium, you might sign a contract in a location that doesn’t serve your career or family goals. Furthermore, walking into an Australian clinic without a deep understanding of the Medicare Benefits Schedule (MBS) means you will under-bill, leaving significant income on the table.

DPA classification applied to all Doctors who have completed medical school overseas, with some exemptions for NZ doctors. As a non-GP specialists, or GP Fellow you will need to be located at an area of need or rural location for up to 10 years unless you have an exemption.

Federal Government

The federal government funds Medicare, providing rebates for GP visits, allied health services (chronic disease and mental health), pathology, radiology, and specialist appointments. It subsidises prescriptions through the PBS, supports veterans’ health via DVA, and contributes to public hospital funding. Additional responsibilities include aged care, immunisation programs, Primary Health Networks, regulating private health insurance and therapeutic goods, funding medical research, and responding to national health emergencies.

State Governments

The state government manages and funds public hospitals, community mental health services, public dental clinics, ambulance and emergency services, and patient transport schemes. It also delivers preventive health services, such as newborn and breast cancer screening, regulates food safety, and oversees infectious disease control and health promotion campaigns.
